The 2026 cut and the Healthy DC Plan

Medicaid Director Letter 25-02 lowered the income limit for parent/caretaker relatives (from 216% + 5%) and childless adults (from 210% + 5%) to 133% + 5%. Adults whose income was above that line lost Medicaid on December 31, 2025.[state]

Residents 21–64 between 138% and 200% of the poverty line who cannot get Medicaid or employer coverage can use the Healthy DC Plan through DC Health Link, which covers primary and specialist care, hospital and emergency care, prescriptions and behavioral health; people who lost Medicaid in the cut were enrolled automatically.[state]

Children and pregnancy

DC covers children of every age and pregnant residents to 319% of the poverty line, financing the upper band with CHIP funds instead of running a separate CHIP; from-conception coverage reaches the same level.[MACPAC]

Pregnancy is not affected by the cut

The January 2026 reduction applied only to parent/caretaker relatives and childless adults. Pregnant residents still qualify to 319% of the poverty line, and DC’s 12-month postpartum extension, approved June 16, 2022, keeps them covered for a year after birth.[state][MACPAC][KFF]
